Objective: An estimated 25% to 50% of all individuals with bipolar disorder will make a suicide attempt. Converging and recent reports have confirmed the effectiveness of lithium prophylaxis in preventing suicide. Lifetime rates of suicide attempts among patients with bipolar I disorder were compared to rates during 18 months period of pharmacotherapy and adjunctive social rhythm therapy.

Method: The study group consisted of 190 patients with bipolar I disorder who entered the maintenance therapies in bipolar disorder program between April 20 , 1998 , and august 10 , 2003 . Subjects were treated with primarily lithium pharmacotherapy and with social rhythm therapy (learn to regularize their daily social routines; i , e , sleep - wake cycle, exercise habits , mealtimes ). Data on prior suicide attempts were obtained retrospectively from interviews with patients or their family. Data on suicide attempts during the clinical trial were collected systematically throughout the protocol.

Results: Seventy - nine attempts were identified to have occurred before the patients entered the protocol (2.2 per 100 person - months). Patients experienced a 19 – fold reduction during maintenance treatment. Statistical analysis showed that reductions were significant in the treatment phase, compared with the pretreatment phase.

Conclusions: Treatment with pharmacotherapy and adjunctive psychosocial intervention can significantly reduce suicidal behaviour in patients with bipolar I disorder.
