The natural course of Bipolar Disorder is characterized by marked severity of acute affective episodes, followed by high rates of relapse and recurrence. Recent epidemiological data provide evidence for a prevalence of more than 6% in the general population. Moreover, Bipolar Disorder may be misdiagnosed as Unipolar Depression when exploration of symptoms is not adequate enough. In consequence, improving the quality of clinical assessments remains a challenge towards a more exact diagnosis and a more accurate follow-up, essential to achieve an effective treatment. The objective of COPE-Bipolar.COM is to improve that quality and to supply an exportable data base to be used for research purposes. It was originated in the Department of Psychiatry at Erasmus Hospital in Brussels and is intended to be used by care givers involved in the management and treatment of Bipolar Disorders. It consists of a computerized tool for Clinical Outcome Measures and is part of the COPE-Bipolar program (Clinical Outcome and Psycho Education in Bipolar Disorders). COPE-Bipolar.COM is composed of the following modules: Demographic Data, the MINI International Neuropsychiatric Interview, Current and Previous Psychotropic Treatment, Side Effects, Somatic Comorbidity, Family History of Psychiatric Disorders, Severity Scales, Quality of Life and Functioning and New Mood Episode. This software has been translated in 4 languages and all the modules are used for the first assessment at program inclusion. Only relevant modules are completed during follow-up. Furthermore, COPE-Bipolar.COM has been designed to easily allow the creation of Private Modules for each centre to add questionnaires, scales or other documents for clinical as well as for research purposes. All the acquired data are easily gathered and automatically exported to an Excel data sheet to be then analyzed as usual by any statistical software.
Baseline and outcome interviews appear as structured assessments. Data has in consequence a definite and highly organized structure. Currently, data on 68 Bipolar (BP) patients (36 women and 33 men) and 58 Unipolar (UP) patients (41 women and 18 men) have been entered in the database at Erasme Hospital. The mean age of both groups of patients is not significantly different (BP=45 years old; UP=46 years old). Data-analysis will look at clinical differences between Bipolar I, Bipolar II and Unipolar patients regarding diagnosis, demographic data, severity, treatments, somatic comorbidity, familial antecedents and functioning.
The Program is now being implemented in different European centres. Local studies can be achieved in those centres that use the software since they are able to individualize the program by running the modules they create to analyse different topics they might be interested in. Similarly, Multicentre studies can be performed, as the central data base can be shared if wished.
