Many authors have reported an increased prevalence of manic vs. depressive episodes in several countries. It has been suggested that a separate entity exists within bipolar disorder (BD) which is unipolar mania (UM) (Abrams R, 1979; Shulman KI, 1994).

The objective of our study was to investigate the prevalence of UM in a sample of 129 patients diagnosed with BD. They were retrospectively assessed for the whole duration of illness (mean 17 years).
37.5% of patients had UM, with male overrepresentation. The total number of affective recurrences was higher in the BDI group, even when controlled for duration of illness. But this difference was no more significant when taking into account only manic episodes.

The age of onset was higher in the UM group (27 vs. 23 years). The seasonal distribution of births was different, with a spring peak in may for BDI and a summer peak in august for UM.

In spite of methodological restrictions (mainly the underdiagnosing of depressive episodes), some data suggest the existence of UM as a separate subgroup within bipolar disorder.
