Previous studies have shown abnormal responses in BD patients to positive and negative facial emotional expressions, particularly in ventral striatal, thalamic, hippocampal and prefrontal cortex. These regions are also implicated in declarative, non declarative memory and attention. Many patients with BD also showed attention deficit and some authors refer an ADD subgroup as a ring of fire were excessive activation of parietal, temporal and frontal regions is observed and suggest that it can be related with BD. Methods.- In this study we analyzed 14 patients with diagnosis of bipolar I affective disorder (DSM-IV), 22 patients with ADD (DSM-IV) and 26 healthy control subjects (CON) both sexes, between 20 to 40 years, similar years of education. The patients were stimulated with viso spatial memory , attention tasks and expressions of fear, sadness, happiness , anger and neutral faces, using Stim and the electroencephalographic activity was acquired with Synamp 1 equipments and Neuroscan 4.3 software (Neuroscan co.) using 22 electrodes according with 10-20 international system, the event related band power (ERBP) (0.1-52.1 hz), face locked, was quantified and compared between groups in 0-2000 ms segment after the stimulus, using t-test for comparing groups and z- transform for comparing individual patients with respect to controls. Results .- During the facial expression test, the BD group showed a significant (p<0.001) excess of power in prefrontal, frontal and temporal regions with respect to CON and ADD groups, no differences between CON and ADD groups were observed,(t-test). During the attention task, the BD group showed excessive power in frontopolar regions with respect to controls in 8 patients, low power in 2 patients and normal in 4. All patients with ADD showed low power with respect to the CON group in frontopolar regions (z-transform). In memory task, 10 BP patients showed very high power in frontopolar and frontotemporal regions, 2 showed low and 2 normal levels. In the ADD group it was detected normal in 14 and low in 8 patients with respect to the CON group in the mentioned areas (Z-transform). Conclusions.- According to the previous results, the BD patients responded excessively to emotional stimulus (92.8 %) , 71% to spatial memory and 57 % in attention task, suggesting that the excess of activity of the amygdale is frequently present also in the hippocampus and basal ganglia, while in ADD the hipoactivity is usual in the basal ganglia and sometimes also in the hippocampus and usually normal in the amygdale.
